How to get involved!

SHARE- Delegations Application and Instructions

Application form for SHARE delegations to El Salvador:

1. Applicants should save your application form as "Your Name Application," i.e. "Jane Smith Application."
2. Fill out all forms completely. Applicants under 18 years of age must have legal guardian fill out Parent Permission Form.
3. Save form.
4. Email form as attachment to Laura Hershberger, SHARE's Grassroots Coordinator at laura@share-elsalvador.org.
5. Print form.
6. Sign and date Liability Waivers. Applicants under 18 years of age must have legal guardian sign and date Parent Permission Form as well.
7. Fax or scan the following items to your Delegation Coordinator: completed application, Liability Waiver (signed and dated), copy of photo page of passport, and Parent Permission Form (only if under 18 years of age). Send scanned items to delegations@share-elsalvador.org, or fax items to (011) (503) 3361-2352.
8. You should receive an email confirmation from the SHARE office that your application was received. If you do not receive this confirmation, please inform your delegation leader.
